All library accident reporting will be handled by the Confidential Personnel Assistant. If an
accident occurs when the Confidential Personnel Assistant is not available, forms are
available at the Information Desk and should be completed and turned into the
Administration office. A form should be filled out immediately for anyone injured on library
property.
Accidents occurring due to a dangerous physical condition (such as wet floors, ice on the
sidewalk, etc.) should be reported immediately to County Maintenance by staff so the
situation can be remedied.
If an accident occurs in a branch, contact the Confidential Personnel Assistant or Customer
Services Team Manager via telephone, and they will fill out the form from information you
have over the telephone.
If the injured person wants a doctor or ambulance called - the call should be made by them
or at their request. Judgment must be made in case of serious injury or if an injured person
is disabled.
After giving necessary assistance, staff must obtain person's name and telephone number,
give needed assistance, and report all information gathered to the Administration Office as
soon as possible.
